Is there no box to input your email address above?

It’s probably because your web browser is Safari, and your cookies aren’t enabled. Safari blocks cookies by default, and our client relationship management system needs cookies in order to work properly. Here’s how to fix it:

• Check your browser setting and make the following adjustments if necessary:
  Choose Safari (top left of screen) -> Preferences -> Privacy -> Cookies and website data: check Always Allow
• Refresh the page
